If applying for the FMLA provided maternity leave, you need to have worked at least 1250 hours in the past 12 months with the employer to be eligible to apply and your employer must employ at least 50 employees for 20 weeks in under 75 miles of the worksite.

You also need to state the timeline of your leave and why you will be taking the days off along with how much time you will be missing at work.
